Tattoo Consultation Form: Fields & Brief Example
A tattoo consultation form collects the details an artist needs to assess a request before discussing a quote or appointment. Start with the idea, placement, approximate size, style, references, budget, timing and a way to reply. Keep uncertain answers visible so the artist can follow up.
Eight fields for a useful tattoo request
Use these prompts as a starting template. Let clients describe an idea in their own words and mark details they have not decided. A budget is a preference to discuss; a requested date is availability to review.
- Idea and meaning
- Describe the subject, what matters about it, and any elements to include or avoid.
- Body placement
- Name the body area and side. Mention existing work that the new piece should sit beside or cover.
- Approximate size
- Give a height and width in centimetres or inches. Allow ‘open to advice’ when the client needs help.
- Style and colour
- Describe the preferred style, black and grey or colour, and how much creative freedom the artist has.
- Reference images
- Attach references and explain the useful detail in each, such as composition, line weight or mood.
- Budget and currency
- Share a comfortable range and its currency. The artist confirms pricing after reviewing the request.
- Timing and availability
- Include preferred days, travel dates or flexibility. Submitting a request does not reserve a session.
- Name and contact
- Provide a name and a reliable reply channel. Explain how the studio will use the contact details.
A sample client brief
Illustrative example, not a client record or a price quote.
A black and grey botanical piece on the outer left forearm, approximately 12 × 6 cm. The client likes the spacing in one reference and the leaf shapes in another, with room for the artist to redraw the composition. Budget: USD 300–500. Available on Fridays next month. Still to confirm: the exact size and how it sits beside an existing tattoo.
Review the request before replying
- Check that the idea fits your style and that the references support the same direction.
- Identify missing or conflicting details. Ask one focused follow-up instead of restarting the whole conversation.
- Confirm the design scope and pricing directly with the client. Keep the agreed next step with the original request.
Keep intake and appointment confirmation separate
This template covers the first creative inquiry. Handle consent, sensitive information and appointment confirmation through the studio’s appropriate process. An AI summary helps organize what the client said; the artist still reviews the request and confirms the final design, price and appointment.
